To better serve our clients and their broad geographical locations, we are a "virtual office". With greater convenience and time-saving flexibility, we are able to work with people via telephone and video conferencing while maintaining high-quality, individualized care.

NEW CLIENTS
New Clients will receive (via email) the appropriate New Client Intake questionnaire shortly after making an appointment. It is critical that these forms be completed, signed and emailed or faxed back PRIOR to your appointment, so please allow enough time for this process when scheduling an Initial Consultation. Unfortunately, we will not be able to proceed with any Initial Appointment if the paperwork is not signed and returned in time. Video is required for the initial session for all New Clients.
ESTABLISHED CLIENTS - with Dr Sullender
Established clients who have multiple lab tests to review, new health issues to address, or have complex case histories, should select the Comprehensive Appointment for the extra time. Please be sure to fax or email any lab tests you have to include in your upcoming appointment. Fax (603) 546-7211 (available 24/7). If you are a parent making an appointment for an under-age child, please enter the Child's name for the appointment.
ACUTE CARE APPOINTMENTS with Dr. Sullender are typically available on short notice and will be scheduled in as quickly as practical. Please send an email with ACUTE CARE APPOINTMENT in the subject line with a brief explanation of what the issue is, your primary symptoms, and the best phone number to reach you. This appt is appropriate for help with colds, flu, injury, sore throats, and other self-limiting conditions. If you have an EMERGENCY, please call your primary care provider.
